The Quarterly Report Creation screen allows you to submit to DOLI Montana Employers Unemployment Insurance (UI) Quarterly Wage Report - Form UI-5 for an employer. This screen can be accessed from the WOW Main Menu screen. As long as your employee wages for the quarter have been entered correctly, WOW will calculate your UI Return for you! WOW will also store your return on a database, so that you can review any UI reports previously submitted on the WOW system.
This Screen has the following sections:
This section displays information for the employer that was selected on the WOW Main Menu screen, so that you can easily keep track of which employer you are currently working on. It cannot be modified.EMPLOYER

FEIN field
WOW displays the FEIN (Federal Employer Identification Number) of the selected Employer.UI Acct No field
WOW displays the Montana UI Account Number of the selected Employer.Business Name field
WOW displays the Business Name of the selected Employer.This section displays two dropdown boxes, and a button.
Year dropdown box
Click in this box and select the Year for which you wish to create a UI return, from the list of available choices.Quarter dropdown box
Click in this box and select the Quarter for which you wish to create a UI return, from the list of available choices. Because UI was moved from the Department of Revenue to the Department of Labor & Industry in 2004, employers will not be able to file reports prior to 1st quarter 2004.Calculate Report button
After you have selected the desired values in the Year and Quarter dropdown boxes, click this button to have WOW calculate your UI report values for the selected year and quarter from the employee wages entered on other screens in WOW. After clicking this button, WOW displays the calculated values for this quarter in the "REPORT INFORMATION" section.NOTE: If you have already submitted a return via WOW for the selected year and quarter, when you calculate a new report, it will be automatically marked as an Amended Return. You do not need to do anything differently to create an Amended Return. Because UI was moved from the Department of Revenue to the Department of Labor & Industry in 2004, employers will not be able to file reports prior to 1st quarter 2004. This section displays ten WOW-calculated UI report values, and three input boxes.
WOW-Calculated Values
Total Wages field
WOW calculates and displays the total quarterly UI wage amount for all employees (for the selected employer) from the Input Employee Wages screen.If the value appears incorrect, click on the "Cancel this report, go to Wages screen" button at the bottom, and re-check the wage amounts you have entered for your employees in this quarter in the Input Employee Wages screen.
Excess Wages field
WOW calculates and displays the total quarterly UI Excess wage amount for all employees (for the selected employer). This value is based on the Taxable Wage Base amount for the selected year (for 2006, this value is $21,600). Any wages paid to an individual employee that exceed this amount in a calendar year are considered Excess Wages.If the WOW-calculated value appears incorrect, click on the "Cancel this report, go to Wages screen" button at the bottom, and re-check the wage amounts you have entered for your employees in this quarter, and all prior quarters in the same year.
Taxable Wages field
WOW calculates and displays the total taxable wage amount for this quarter (for the selected employer). This value is calculated by subtracting the Excess Wages amount from the Total Wages amount.Total Tax Rate field
WOW displays the Total Tax Rate (as a percent) for the selected employer. This value is retrieved internally by WOW from the selected employer's internal DOLI account. For more information, call DOLI at (406) 444-6963 or e-mail the WARP Administrator (mailto:arose@mt.gov).Computed Tax field
WOW calculates and displays the gross tax due for the selected employer, for the selected year and quarter. This value is calculated by multiplying the Taxable Wage amount by the Total Tax Rate percentage.Payments Made field
WOW displays the sum of all UI payments made via WOW for the selected year and quarter, for this employer. If no UI payments have been made yet, for this employer and quarter, the value will be zero.If the WOW-calculated value appears incorrect, click on the "Cancel, Remove this report" button at the bottom, then return to the WOW Main Menu, navigate to the Payment History screen, and check the payment amounts you have entered for this employer in this quarter.
Allowable Credits field
WOW displays the Allowable Credit amount for the selected employer only if the employer calls DOLI.Net Payment Due field
WOW calculates and displays the net tax due for the selected employer, for the selected year and quarter. Please pay this amount to the Department of Labor and Industry. This value is calculated by subtracting the Payments Made amount and Allowable Credit amount from the Computed Tax amount. Any penalties and interest are added to this amount (see "Penalties" and "Interest" below).Submit Date field
This field displays the date that this return was submitted on WOW. After the "Submit" button is clicked, WOW automatically fills in this Submit Date field with the current date.Confirm Num field
This field displays the WOW Confirmation Number assigned to a submitted UI return. After the "Submit" button is clicked, WOW automatically fills in this Confirmation Number field with the assigned value.Employer-Entered Values
Employee Count 1 field
Enter the number of employees on the the selected employer's payroll on the 12th day of the first month in the selected quarter. This value is required before a UI return can be submitted to DOLI.Employee Count 2 field
Enter the number of employees on the the selected employer's payroll on the 12th day of the second month in the selected quarter. This value is required before a UI return can be submitted to DOLI.Employee Count 3 field
Enter the number of employees on the the selected employer's payroll on the 12th day of the third month in the selected quarter. This value is required before a UI return can be submitted to DOLI.Penalties field
WOW displays the penalty amount for the selected employer for selected current quarterly report only.Interest field
WOW displays the Interest amount for the selected employer for the selected current quarterly report only.At the bottom of the screen are several buttons and hyperlinks.
IMPORTANT NOTE: Department of Labor and Industry will not receive your report until you click on one of the two "Submit Report..." buttons.Submit Report, Stay on this screen button
If you are satisfied that the displayed UI Report values are correct, click this button to save and submit this UI Return to DOLI, and redisplay this same screen when WOW is finished saving and submitting your UI return.Submit Report, go to Payment screen button
If you are satisfied that the displayed UI Report values are correct, click this button to save and submit this UI Return to DOLI, and then navigate to the Add\Modify Payment screen when WOW is finished saving and submitting your UI return.Cancel, Remove this report button
If you do not wish to save and submit the displayed report to DOLI, click this button to cancel and remove the displayed report, and redisplay this same screen.Cancel this report, go back to Wages screen button
If you do not wish to save and submit the displayed report to DOLI, click this button to cancel and remove the displayed report, and navigate to the In put Employee Wages screen.View a Printable Report link
If you wish to have a copy of your UI Report, click this link after you have submitted your report to DOLI. WOW will open a new browser window with your current UI Report displayed in Adobe Acrobat. You must have the Adobe Acrobat Reader software in order to use this feature.NOTE:You must submit the report before you can view a printable copy. If your report is not submitted before you click this link, you will see a blank window.
