Information required:
                    1)  Transmitter FEIN number and Business Name
                    2)  Employer FEIN, Customer ID, Name
                    3)  Contact name, phone number, and email address
               If you are interested in making electronic payments, also provide:
                    4)  Bank routing number for each Employer
                    5)  Bank account number for each Employer
                    6)  Bank name
